LOLA Worker’s Survey

Welcome!
This Worker’s Survey was developed as part of the Landscape Ontario COVID-19 and Labour Assessment Project (LOLA), supported by the Ontario Ministry of Agriculture, Food & Rural Affairs (OMAFRA). Your responses to this survey are totally confidential, and your information is anonymous. All fields marked with an asterisk (*) must be completed before your survey can be submitted. Survey Purpose:
- This survey is a place for you to tell us how the COVID-19 pandemic has affected you, the worker
- An opportunity to voice your concerns regarding potential exposure to COVID-19 in nursery production, through daily activities and after work hours
- To give you the chance to share your comments and ideas on how the industry can minimize your exposure to COVID-19.
- By completing this survey, you are helping to make your workplace safer.
After Completing the Survey:
After you Submit your survey, we'll show you your completed survey. You can print on paper (or to a PDF file) to share with your employer or Health & Safety Committee. We will also include links to Public Health and general COVID-19 information. Data from all worker surveys will be amalgamated and used to provide recommendations to Public Health, OMAFRA and nursery owners in Ontario with the goal to improve your work experience.
